_________________________________________________(“Artist”) understands and agrees that Lakewood United Methodist Church (“LUMC”)located at 1390 Brentwood St., Lakewood, CO 80214 is allowing me to display in the LUMC Gallery, the items described on Exhibit 1 (“Artwork”) from _________________________________ through ______________________________(”End Date”) for the purpose of the exhibition _________________________________________________(title).
Artist represents that the Artist is the sole legal owner of all rights, titles and interests in the Artwork and will indemnify and hold LUMC harmless from any claims by third parties related to infringement of intellectual property rights.
LUMC is not liable for any damage to the Artwork and carries no insurance for loss or damage to Artwork in the Gallery, nor does LUMC assume any liability for the same. Artist should provide Artist’s own insurance coverage.
Artist grants permission for LUMC to reproduce Artist’s name, information describing the Artwork, representations of the Artwork and any other information provided by the Artist for the purpose of display, promotion and publicity either now or in the future. The Artist grants LUMC a license publish, promote, photograph and video the Artwork in print, internet, television or other media.
Artist is solely responsible to display, pack, transport, and provide for the return of the Artwork at Artist’s own risk and expense. Artist agrees to transport the Artwork before the Opening Date and shall be removed it from the Gallery’s within 3 business days following the End Date above, unless otherwise noted in a written agreement between the parties. Artwork left over 30 days beyond the End Date be automatically transferred to, and become the property of, LUMC.
The Artist understands and agrees Artist has read this Agreement and confirms that Artist is bound by its conditions. Artis releases LUMC from liability for loss or damage to the Artwork . The Artist assumes all responsibility and obligation for any loss or damaged item(s).
The Artist is responsible for notifying LUMC immediately of a change in Artist’s contact information. In the case of a change of legal ownership the Artwork, the Artist and the new owner must notify LUMC immediately and provide proof of ownership.
